How to change a patient or client name

There are two different ways you can change a patient’s name in Twofold, depending on your goal:

  1. Change the patient assigned to a specific note (for example, if a note was generated under the wrong patient).

  2. Edit the patient’s actual name in their profile (for example, correcting a spelling error or updating a last name).


Option 1: Change the Patient on a Given Note

If you need to reassign a note to a different patient:

  1. Open the note you want to update.

  2. Click the three-dot menu in the top-right corner of the note screen.

  3. Select Change Patient from the dropdown menu.

  4. A pop-up window will appear:

    • You can select an existing patient from the list.

    • Or create a new patient if the correct one does not already exist.

  5. Confirm your selection. The note will now be associated with the updated patient.

✅ Use this option when a note was accidentally generated for the wrong patient.


Option 2: Edit a Patient’s Name in Their Profile

If you need to permanently update a patient’s name in your system:

  1. From the left-side navigation, go to the Patients table.

  2. Search for and click on the patient whose name you want to edit.

  3. You’ll be taken to the patient’s profile page.

  4. In the Overview tab, locate the Pronoun and Name field.

  5. Update the patient’s name as needed.

  6. Click Save to confirm your changes.

For example, if you have a patient listed as Paul Hart but need to update it to Paul Do, you can make the correction directly here.

✅ Use this option when you want to change or correct the official name of a patient in your records.
