Editing and Finalizing Notes

How to review, edit, and finalize your notes before copying them to your EHR.

Navigating to Your Note

Once a note has been generated by Twofold Health’s AI, you can easily access it from the Notes List on your dashboard. Each note entry will display the following details:

  • Note Title: The title of the note as recorded.

  • Patient Name: If provided during the session, the patient’s name will appear here.

  • Note Date and Duration: The date the note was created and the duration of the session.


Editing Your Note

The note is structured according to the template you selected before recording, with each section generated to match the specific documentation needs of that template. Here’s what you can do:

  • Edit Sections: Click on any section of the note to edit the text. This allows you to refine the details, add missing information, or adjust the language as needed.

  • Copy and Paste: You can easily copy the content from any section of the note and paste it into your Electronic Health Record (EHR) system or any other document you need.

  • Copy All: Use the Copy All button at the top of the page to quickly copy the entire note to your clipboard for pasting into your EHR or other documentation systems.

  • Magic Edit: Instead of editing text by hand, you can tell Twofold's AI exactly what to change. Click the Magic Edit button at the top of your note, type your instructions in the popup, and click Submit. Twofold regenerates the note with your changes applied. For best results, be specific about which section to change, and give one instruction at a time. See the full list of Magic Edit examples below.


Magic Edit: examples and tips

Magic Edit works best when you're clear about what to change and where. Here are the most common ways to use it.

Add information to a section: "Insert a review of systems covering all 14 systems in the Subjective section." "Add these lab results to the Objective section: CBC normal, CMP within range, TSH 2.1." "Expand the Assessment and Plan to include the discussion about medication side effects." "Include the specific discussion about the client's sleep concerns in the Subjective."

Add medical codes: "Include ICD-10 codes in the Assessment and Plan." "Add CPT codes to the note."

Change names and pronouns: "Change pronouns from they/them to she/her across the entire note." "Replace John with Jon throughout." "Change the patient to the client's name everywhere."

Remove information: "Delete insomnia and anxiety from the problem list in the Assessment and Plan." "Remove the medication management section entirely."

Restyle a section: "Rewrite the Interventions section in paragraph form instead of bullet points." "Make the Plan section more concise."

After submitting, always review the updated note to make sure the changes look right.


Additional Actions on the Note Page

Beyond editing, you can access additional note management features through the three-dot menu (⋮) in the upper right corner of the note page:

Edit Time, Duration, or Modality

To edit session details, click the three-dot menu (⋮) in the upper right corner of the note page and select Edit time, duration or modality. You can adjust the session date, time, and duration after the note has been generated. Additionally, you can change the modality between 'In person' or 'Virtual'.

Mark as Unread

To flag a note for later review, click the three-dot menu (⋮) in the upper right corner of the note page and select Mark as Unread. If you need to revisit a note later or want to flag it for further review, this helps you keep track of notes that still need attention.

Print

To print your note, click the three-dot menu (⋮) in the upper right corner of the note page and select Print. This allows you to quickly print your note directly from Twofold Health for your physical records or to provide to other healthcare providers.

Download PDF

To download your note as a PDF, click the three-dot menu (⋮) in the upper right corner of the note page and select Download PDF. This creates a PDF file for easy storage, sharing, or uploading to external systems. This is particularly useful for maintaining backup records or sharing with colleagues.

Change Patient

To reassign a note to a different patient, click the three-dot menu (⋮) in the upper right corner of the note page and select Change Patient. If a note was accidentally assigned to the wrong patient, this option allows you to reassign it to the correct patient.

View Patient

To access a patient's complete record, click the three-dot menu (⋮) in the upper right corner of the note page and select View Patient. This takes you to the patient's profile where you can see all associated notes and session history in one place, providing a comprehensive view of your documentation for each patient.

Regenerate Note

To create a new version of your note, click the three-dot menu (⋮) in the upper right corner of the note page and select Regenerate Note. This is useful if you want different phrasing or after making adjustments to the recording. This will create a new version of the note based on the same session.

Change Template

To switch to a different note template, click the three-dot menu (⋮) in the upper right corner of the note page and select Change Template. If you decide that a different template would better suit the session, this will regenerate the note according to the new template's structure.

View Transcription

To access the full word-for-word transcript, click the three-dot menu (⋮) in the upper right corner of the note page and select View Transcription. This feature helps review patient conversations and any specific details from the session.

Delete Note

To permanently remove a note, click the three-dot menu (⋮) in the upper right corner of the note page and select Delete. This action is irreversible, so be sure before you proceed.


Rating Your Note

Provide feedback on the accuracy and quality of the generated note by rating it at the bottom of the page. Your feedback helps improve the AI model, ensuring better results for future sessions.


Finalizing Your Workflow

Once you've edited and finalized the note to your satisfaction, you can proceed to integrate it into your EHR or any other system you use for record-keeping. You can:

  • Copy and paste individual sections

  • Use the Copy All button for the complete note

  • Download as PDF for backup or sharing

  • Print for physical records
